    Occupational Therapist (OT) - Contracted Home Health Position in Almonte, California

    This opportunity is designed for an independent contractor to serve the Almonte area specifically.

    FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver exceptional care and work with adults of all ages.

    The Occupational Therapist in Home Health will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment and evaluation, the Occupational Therapist will determine a treatment plan, perform interventions to improve patient wellbeing, and evaluate progress.

    Key Responsibilities:

    • Provide services defined by state laws governing occupational therapy practice, in accordance with the plan of care, and in coordination with the healthcare team.
    • Lead patients in therapeutic, creative, and self-care activities to enhance functioning and safety.
    • Observe, record, and report patient response to treatment and changes in condition.
    • Instruct patients, families, caregivers, and the healthcare team in areas where they can assist the patient.

    Requirements:

    • Ability and enthusiasm in working with all ages
    • OCCUPATIONAL THERAPIST LICENSE AND REGISTRATION BY THE STATE REQUIRED
    • Completion of an accredited Occupational Therapist program
