CloudCare
To bring health care delivery for insulin dependent patients to the next level, Diabeter has developed CloudCare.

CloudCare is an integrated patient population management solution, which enables care providers to remotely monitor and manage all their insulin dependent diabetes patients (hereinafter referred as the "Solution" or "CloudCare").

The Solution carefully manages data from different sources.

It combines patient specific clinical information and connects this with device information to create an automated risk stratification of the total patient population of the hospital.

The Solution integrates seamlessly with the existing clinical workflows allowing an easy visualization of device data from all manufacturers (Brand agnostic).

Via a patient app, both the HCP and the patient have real-time insights on daily glucose outcomes. This common ground of outcome transparency empowers patients to self-manage their condition and improve / sustain their outcomes.

**A Day in the Life
**As a Medtronic Cloudcare Technician you will closely work with the Diabetes Team of the Hospital and the Diabeter organization from Medtronic. Your workplace is at the hospital.


In your role as Cloudcare Technician, you work with the Cloudcare solution performing the daily triaging of the incoming Glucose data.

With this non clinical triaging service you support the Diabetes Team of the assigned Hospitals to identify patients who need their care the most.

Next to the triaging service you are instrumental in taking a coordination role between device manufacturers and the Diabetes Team.

You also support patients in technical questions related to the Patient app or the Cloudcare uploading functionality.

You are assigned to clinical related administrative tasks related to patient care, e.g., taking on responsibilities for incoming messages at the patient portal and acting on the non clinical elements.

You are also assigned to train new team members or your could be involved in Cloudcare related studies.

The role is varied and dynamic, and you are asked to provide your inputs to make best use of Cloudcare within the Hospital as well as towards the Diabeter organization to influence the shaping of the offering.


Next to a high interest in digital health delivery, you are used to take over administrative tasks, you have good analytical skills and a structured way of working.

Teamwork and establishing a healthy collaboration with the Hospital internal and also the Diabeter organization are vital elements in succeeding in this role.

As part of the Medtronic Diabeter organization, you should be able to:

  • Work independently and collaborate with your leader mostly remotely.
  • Share and communicate determined best practices proactively.
  • Support the development of the solution by providing inputs from your daily practice
  • Codevelop training material.
  • Being flexible to work on different potentially also newly upcoming tasks and assignments.

Key Responsibilities Include

  • Daily monitoring of incoming glucose data from patients and triaging those data based on the defined clinical protocol (the Cloudcare Technician does not take clinical decisions. The patient triaging requires close collaboration with the Diabetes care team of the Hospital).
  • Onboarding support of new patients to Cloudcare and ensuring regular data uploads from patient side.
  • Being responsible for a proper dataflow and working together with patients and the IT of the Cloudcare provider in case of missing data or inaccurate data.
  • Providing proactive inputs and userfeedback through the Cloudcare IT support portal.
  • Working on Cloudcare training material and train new people in the Diabetes department as needed on the Cloudcare solution.

Non Cloudcare related activities:

  • Acting as a contact person between the hospital and the Diabetes technology providers.
  • Supporting the hospital management in providing measures / statistics.
